that provides confirmation of delivery. 

This award is given to the College Athletic Trainer or the Collegiate Athletic Training Staff that has risen to the occasion or continually acts to improve the quality of life of fellow collegiate athletic trainers and others. When nominating an individual or staff, examples can include working to improve the quality of life for others such as helping staff members relative to family, time-off, salary compensation; implement plans to reduce workload and increase personal time, improve work place environment or other examples which inspire individuals to improve the qualify of life for themselves and/or others.

Award

CATS will award winning nominee(s) the amount of $1500, sponsored by PRO.

Criteria for the CATS Quality of Life Award

• Only active CATS members are eligible for the award.  Non-CATS members must be approved by the CATS Board of Directors.
• Nominations can come from anyone associated with the individual or staff. 
• Nominations may be reconsidered towards the next award and will remain active for two years.
• Nominations can be retroactive.

Please submit the following:

• Completed nomination form with required signature(s).
• Minimum two letter(s) of recommendation describing how nominee improved quality of life of fellow collegiate athletic trainers. 
• Nominations may include any additional documentation nominators would wish to include to further support nomination. 
• Nominations shall be submitted to the CATS Family Scholarship Committee by annual deadline, April 1st, for consideration.

Submitted nomination form must be completed in its entirety.  Incomplete applications will not be considered.  Award recipients will be notified by CATS President or designee.  Award announcement will be made at the CATS Spring Symposium or Annual Symposium.
